WHOO HOO!!!!! Okay, I can now let you all know that one of my "hobbies" is entering sweepstakes.....because I just won a GRAND PRIZE!!! Here what I won>>>>
As the Grand Prize winner, you will receive a trip for two (2) to Baltimore, MD. The trip includes round trip coach airline tickets for two (2) from the major gateway nearest your home to the major gateway nearest Baltimore, MD; two (2) night hotel accommodations (double occupancy); two (2) tickets to see LeAnn Rimes in concert; and a “Meet & Greet” for (2) two with LeAnn Rimes. Travel will take November 16th through November 18th, 2007. You will be responsible for all federal, state, and local taxes on the prize package.
Enclosed with this letter are an Affidavit of Eligibility & Release and a W-9 form. The Affidavit of Eligibility and Release is to confirm that you have complied with all rules and regulations, that you are 18 years or older, that you are not an employee of any contest sponsor, and that you fully accept the prize. It is important that the forms be completed, signed, notarized, and returned to me by Tuesday, November 6, 2007. Please remember to make a photocopy of the completed affidavits for your records.
